If BOX A on the face of your Violation Notice is checked, YOU MUST APPEAR IN COURT at the date, time, and place shown. If no date is shown, the Central Violations Bureau ("CVB") will mail you a Notice to Appear indicating the date and time you must appear in court. If your Violation Notice does not tell you when to appear in court, and you do not receive a "Notice to Appear" within 45 days of the date the Violation Notice was issued, call the CVB at (800) 827-2982.

YOU MUST NOTIFY THE CVB IN WRITING OF ANY CHANGE IN YOUR ADDRESS THAT OCCURS AFTER YOUR VIOLATION NOTICE IS ISSUED, OR YOU MAY NOT RECEIVE YOUR NOTICE TO APPEAR. If you fail to appear at a scheduled court hearing, the court may issue a warrant for your arrest. Send your notice of an address change to the Correspondence Address listed below.

If BOX B on the face of your Violation Notice is checked, YOU MUST DO ONE OF THE FOLLOWING:

1. PAY A FIXED SUM IN LIEU OF APPEARING IN COURT. If you wish to end your case without appearing in court, submit payment for the "total collateral due" amount shown on the Violation Notice by check, money order, or credit card. Use the envelope from the Violation Notice and include the stub with your payment. DO NOT SEND CASH. Write the violation number and location code that appear on the top front portion of the Violation Notice on your check or money order and make it payable to the "Central Violations Bureau". Alternatively, you may submit payment online at www.cvb.uscourts.gov.

   By paying the amount due you may be admitting to a criminal offense and a conviction may appear in a public record with adverse consequences to you. You have a right to know more about the charge against you and may obtain a complete statement of the charge by calling the CVB at (800) 827-2982. By paying the amount due you waive your right (1) to contest this violation notice, (2) to a trial, and (3) to be represented by counsel.

   If you are charged with a motor-vehicle violation, your payment may be reported to your state's motor-vehicle or driver-licensing agency. As a result, points may be assessed against your driving record, your license or registration may be suspended, and additional fees may be imposed by your state. A driver education course (traffic school) may be an option to avoid these consequences. If you are interested in a driver education course, do not pay the amount due. If you are not eligible to make such a request in advance, you may still be eligible for traffic school, but you must appear in court to make the request.

   If you intend to pay instead of appearing in court, your payment must be received by the date of your scheduled court appearance. You may pay online on the day your court appearance is scheduled, but if you do not pay until after that date, penalties may be imposed and the court may issue a warrant for your arrest.

   OR

2. APPEAR IN COURT. You may appear in court to discuss or contest your violation, or to request traffic school in person for a motor vehicle violation rather than online. If a date, time, and place for you to appear in court are shown on the front of your Violation Notice, you may appear in court at that time. If no date is shown, the CVB will mail you a Notice to Appear indicating the date and time you may appear in court. If your Violation Notice does not tell you when to appear in court and you do not receive a Notice to Appear within 45 days of the date your Violation Notice was issued, call the CVB at (800) 827-2982.

   If you appear before the court and plead not-guilty, a trial will be scheduled at a later date of no less than 30-days at which time you will be given the opportunity to present evidence and witnesses on your behalf.

   If you appear before the court and plead guilty, or are convicted after a trial, the court may impose any penalty authorized within the law, not limited to the "total collateral due" amount shown on the Violation Notice. In addition, the court may impose a special assessment of $5, $10, or $25 for each offense.
